Making Appointments
We see patients by appointment Monday through Friday. We have evening appointments Monday through Thursday until 8 PM. In addition, the office is open every Saturday for blood work and physical testing performed by the nursing staff. Call us at: (973) 942-6005 to schedule.
Appointments
INITIAL VISIT: We encourage you to come in at least 15 minutes before your initial visit. This will give you the time to fill out all the necessary registration material needed by our office. Please make sure you bring your insurance card and proof of address with you. For your convenience, you can save time by downloading these forms in advance from our website and completing them prior to your arrival.
Insurance Cards
Upon every visit, you will be asked to present your insurance card at the time of check-in. This is to ensure that your billing information is accurate. Please be aware if we do not have the current insurance information, you will be responsible for any charges that were incurred at the time of visit.
Co-Payments
Some insurance companies require patients to pay their co-payment when they are seen in the office. It is your responsibility to pay your co-pay at the time of your visit. If a co-payment is not made at the time of visit, the office reserves the right to reschedule your appointment to a later date or assess an administrative processing fee.
Please note for your convenience, our office accepts cash, personal checks and credit cards as forms of payment
Returned Checks
If you choose to pay your co-payment by Personal Check, and for any reason the check is returned from our bank, we will assess a $25 surcharge to your bill. Please note that any surcharges assessed for returned checks or outstanding co-payments must be fulfilled before your next visit.
Canceling An Appointment
Please note that the office requires 24 hours notice for any appointments that require cancellation or rescheduling. A charge of $30 will be assessed for appointments not cancelled within the required 24 hours.
Referrals
The office requires 72 hours notice to issue a referral. This time allows our staff to obtain your doctor's approval, contact the insurance company if necessary, or reach out to the patient if additional information is required. You may call the office or use the Patient Portal on our website to request a referral.

There is always a physician on call 24 hours a day.

For your convenience, parking is available in Wayne Commons complex. Enter building through door on far right of 510 building.
